About Our Formulary

A formulary is a list of covered drugs selected by us in consultation with a team of pharmacists and other health care providers. We will generally cover the drugs listed in our formulary as long as the drug is medically necessary, the prescription is filled at a Symphonix Health network pharmacy, and other plan rules are followed. 

What you pay for your drugs will depend on which “tier” of our formulary your drug is in.  Our formulary has 5 tiers and where the drug is listed depends on whether it is a generic drug or a brand drug, among other factors. For More information about your benefits, including how much you pay for drugs under our plans, please go to Documents to consult your Evidence of Coverage.

Medication Therapy Management (MTM)

Some members of our plan will qualify for our Medication Therapy Management (MTM) program. You will usually qualify if you have certain chronic diseases and are taking a large number of drugs.  If you do qualify for our MTM program, we will notify you in writing and we will explain how the MTM program can help you make sure the medications that have been prescribed for your medical conditions are safe and effective.
